Although the system has been through extensive testing, no system is ever completely free of problems. Please report problems as you find them. Types of problems that you should report are:
System crashes
Error messages
Inability to do something
Not working as it used to
Not working as you expect
Not working as you would like
Please provide as much information as you can
What were you doing when it happened
Detail description of the steps that led up to the problem
Is it repeatable, if so how?
What happened?
Details of error messages (if present)

When the system crashes, you will see an error message, which will look something like this:
